Even starting a conversation about what happens after end of life care is difficult, let alone making plans for it in advance. However, we at Sahar Hospice Care recommend you to take End of Life decisions. Discuss and formalize your Will in advance. Not only will you be in control of your medical care decisions, but your family will be able to honor your last wishes, giving you the peace of mind you so long for.

Advance Directives

This a legal document that lets your family and hospice care provider know how to proceed with your medical care if you are unable to communicate your choice.

There are two components of an advanced directive:

  • Living Will

This section contains your hospice care choices and treatment options. Even when you are unconscious, your wishes will be respected.

  • Power Of Attorney

A person with a power of attorney can make decisions on your behalf. Thus, your appointed person should be a close friend or family member that you trust fully.

Discuss your Living Will with them so they can reassure and confirm your preferences for hospice services.

Considerations

 

The advanced directive must include the following to be accepted in Texas and the rest of the USA:

  • Signed by patient
  • Signed by two witnesses excluding your heirs and hospice care provider.
  • Power of attorney must be notarized.
  • Should be portable; can be presented whenever necessary.
  • May not be applicable from state to state

Coming Into Effect

 

Before Sahara Hospice Care proceeds with your directive of end of life care, two hospice doctors must confirm:

  • You are medically unable to make your decisions
  • You are terminally ill or permanently unconscious per Texas law.
